INSTRUCTIONS FOR PESTICIDE REPORTING LAW: eForm 44-15-27E(Annual Report for Pesticide Sales to Certified Private Applicators) Table of Contents: ----------------- 1) General Information 2) Specific Instructions For Filling Out Eform 44-15-27e 3) County Codes ---------------------- 1) GENERAL INFORMATION: ---------------------- THE PESTICIDE REPORTING LAW (CHAPTER 279, LAWS OF 1996) requires all commercial applicators/technicians to file with the DEC at least annually a report containing information on each pesticide application. Reports are due by February 1 of the year immediately following the reporting year. Violations of the Pesticide Reporting Law are subject to penalties of up to $10,000 and could result in the revocation of your applicator certification and/or pesticide business registration. Read this document for information about what data to enter on the forms. This form is to be used to report all sales of restricted use pesticides or general use agricultural pesticides to certified private applicators. Your report can be transferred to DEC by e-mail, diskette, CD-ROM, FTP protocol or using the "Email Report To Nysdec Via Internet" feature of the software (see the User Guide). PLEASE NOTE: All submissions must also contain the identity text file (IDENTITY.TXT) That was created for you from the registration screen within the software. This file is located in the "C:\PRLYYYY\IDENTITY.TXT" (where yyyy is the reporting year, 2006 for example) directory. -------------------------------------------------------- 2) SPECIFIC INSTRUCTIONS FOR FILLING OUT eFORM 44-15-27E: -------------------------------------------------------- PLEASE NOTE: Ditto marks, the words "SAME AS," or arrows CANNOT be accepted on the electronic forms. Please see the User Guides for information about using the Copy Row and Delete selected Row/s features. Filling out the HEADER INFORMATION: Each time you go from the initial screen (the registration form) into the report form, the appropriate registration information will be copied into the header area of the new form automatically. On Form 25E the COMMERCIAL PERMIT NUMBER and the BUSINESS NAME from the registration screen will be copied over. After they have been moved over, the user can change these fields by positioning the mouse in the field and entering the new value. If entering a new value for the ID number, this is a five digit identification number beginning with an "R," which can be found on your Commercial Permit. This new value will be used when the file is saved. All records in the file will be associated with this value. FOR EACH REGULATED PESTICIDE SALE YOU MUST RECORD: COL 1 EPA REGISTRATION NUMBER - Report the EPA REG. NO., exactly as it appears on the product label. Important: Be sure to use the EPA REG. NO. and NOT the EPA EST. NO. All EPA Reg. Numbers are separated by hyphens/dashes (i.e.: 3125-474-538 or 3125-474). (The EPA Reg. No. Maintenance List can be used, see the User Guides for instructions.) COL 2 PRODUCT NAME - Report the registered product name NOT the active ingredients. COL 3 QUANTITY SOLD - Multiply the size of the container by the number of containers and report the result. Report using decimals; round off to two places, if necessary. Convert fractions to decimals. COL 4 UNITS - (Click in box for automatic drop down list. Use arrow to scroll to appropriate unit). REPORT THE UNIT OF MEASURE APPLICABLE TO THE QUANTITY SOLD. Field may only contain one of the following abbreviations: C (Cup) OZ (Dry Ounces) FL (Fluid Ounces) PT (Pint) GL (Gallons) LB (Pounds) GM (Grams) QT (Quart) KG (Kilograms) TE (Teaspoon) L (Liters) TA (Tablespoon) MG (Milligrams) TN (Ton) ML (Milliliters) COL 5 DATE SOLD - Dates must be entered in the format MM/DD/YYYY where M=month, D=day, Y=year. COL 6 COUNTY CODE - Report the county where the pesticide was applied. (A pop-up list is displayed in this column in versions 2.1.1 or higher of the software. There is also a county code table below.) COL 7 ADDRESS - Report the address where your customers intend to apply the pesticide. If application address is different from billing address, enter the application address (P.O. Boxes are not permitted). COL 8 MUNICIPALITY - Report the village, city, etc., where your customers intend to apply the pesticide. COL 9 ZIP CODE - Report the five digit zip code where your customers intend to apply the pesticide. For applications that cross a county, municipality, township, or zip code boundary, you must enter complete information for each location where the pesticide application is intended.
